A new way is under process for gathering customer information through lead form extension says Google Ads. Advertisers are now able to create lead forms with their own title and description. They can highlight which piece of information the advertiser needs from the customer such as customer name, Email, Phone number, Postal Code.

New Features:
- Background Image:
Advertisers now have the option to upload a background image of their own choice in the lead form. It is clearly shown in the above image.
- Customizable Message:
Advertisers can change the message that appears after the submission of the form according to their choice after the user submits their relevant information.
These new lead form extensions is a beta feature that is available to limited businesses. Google says it is not eligible for some sensitive verticals and sub-verticals.
